    If you join this study, you can learn how to eat better, be more active, and get better sleep. Your sleep environment includes things like how warm or cool your room is, how light or dark it is, how noisy it is, and the air quality. You'll also get information about your blood pressure and later, blood tests that show your risk for heart disease. Being in this study will help us learn the best ways to improve health, especially sleep, in our NYC communities. This can help reduce heart disease and cancer.

    Any adult can join who:
    1) is 20 years of age or older that can provide consent 
    3) speaks English and/or Spanish
    4) has blood pressure that is borderline or high (before enrollment, the study team will conduct a blood pressure measure to confirm)

    The purpose of Sleep2BWell is to find out if improving sleep health in addition to improving diet and physical activity habits will result in better heart health, specifically lower blood pressure, weight loss, and better blood tests of heart health.

    To maintain confidentiality and protect privacy, all collected data and samples will be de-identified with a study number and without any specific identifying personal health information. Only the study team will have access to personal identifiers to contact participants for follow-up. Study data will be stored on password-protected, encrypted devices. Only personnel immediately involved in the study will have access to study files, documents, and specimens.

    24 weeks

    If you join this study, you could get up to $225 for participating. When you sign up, check your blood pressure and weight, and complete the questionnaires, you get $25. If you do a blood test when you join, you'll get an extra $50. If you come to group classes and let us check your blood pressure, weight, and waist size ten weeks later, we'll give you another $50. Then, 24 weeks in, doing those checks again plus another blood test, you earn $100 more. Everyone in the study gets a Fitbit to track activity. Everyone will also get a face mask, ear plugs, an air purifier, and a blue light blocker at the start or end of the study, depending on your group assignment. Suppose you are selected to discuss your experience after the study ends. In that case, that's an extra $50, making your total possible earnings $275.

    Heart disease and cancer risk.

    Researchers will use Sleep2BWell to evaluate the effect of combining sleep health education with ongoing diet and physical activity education may further improve heart health and reduce cancer risk in community settings.

    If requested, you will be provided with results from your blood pressure measurements and all completed blood tests. 

    If you decide to join this study, we will ask you to share some information about your health history and your daily habits. You can fill out the questions online whenever it works best for you. Over six months, we'll check your height, weight, waist circumference, and blood pressure three times. If you're okay with it, on two of those visits, we'll need to collect a small amount of blood (around two tablespoons) from your arm. This is to test for indicators of inflammation and how well your body controls blood sugar. If you agree, we'll keep your blood sample stored in a freezer for possible use in later research.

    You can withdraw by contacting study staff via email at any time during your study participation. If you choose not to volunteer you will not lose any services, benefits, or rights you would normally have.

    Your information may be shared with approved research staff and study investigators, but we will not share information that directly identifies you. We may use stored specimens to measure other heart disease risk factors such as blood sugar, insulin, and other test of inflammation in the body. Also with your permission, data or blood samples may be used by other Weill Cornell or Columbia researchers or researchers at other institutions for research on blood pressure or other conditions. If they are given to researchers who are not researchers on this study, they will only be given in de-identified form. This means that your name and other identifying information have been permanently removed from your data or samples.

    You may or may not receive personal (direct) benefits from taking part in this study. Participation may help you improve your lifestyle habits (sleep health, diet quality, and physical activity). 

    There are a few risks to individuals enrolling in this study. Risks of having blood drawn are soreness and/or a black and blue mark at the site from where the blood is drawn. Sometimes, people feel uncomfortable at the time of the blood draw. Occasionally people feel lightheaded or weak. There is also a small risk of infection whenever blood is drawn. 

    We will do everything we can to keep your data secure, however, there is a risk for breach of confidentiality (although unlikely). Loss of privacy means having your personal information shared with someone who is not on the study team and was not supposed to see or know about your information. The study team plans to protect your privacy. Their plans for keeping your information private are described in the Privacy section of this consent form.

    If you join this study, there is a potential risk to your privacy because technology is being used to track your data. Vibrent Health uses advanced technologies to protect your data, however we cannot guarantee your privacy. If there is a data breach, someone who is not authorized could get information about you. This chance is very small, but it is not zero. There may also be other risks that are not currently known. While we will do our best to protect your data, we cannot remove all risks. In general, there is no additional risk to using the study app beyond the risk you take when using technology in your daily life.

    By using the study app there is a risk you may be identified as a study participant. This may occur if your device is lost, misplaced, or stolen. It may also occur if your identity and information recorded in the study app are otherwise accessed by another person.  

    We have several steps in place to protect your data. Data we collect will be stored on password protected computers. We will limit who is allowed to see information that could directly identify you. We will tell you if there is a data breach that we think could be harmful to you. Your privacy will always be our top priority.
